Non-DDC Student Liability Waiver & Release

By submitting this form, I give permission for my child to participate in the Precision Jazz Master Class at Doylestown Dance Centre on July 1, 2026.

I understand that dance instruction involves physical movement and activity, including but not limited to stretching, jumping, turning, traveling steps, kicks, turns, traveling combinations, and other dance-related exercises. I acknowledge that participation carries the risk of injury, including minor or serious injury.

I certify that my child is physically able to participate in this class and has no medical condition that would prevent safe participation. I agree to inform Doylestown Dance Centre of any relevant medical concerns, allergies, injuries, or limitations before the class begins.

In consideration of my child’s participation, I release and hold harmless Doylestown Dance Centre, its owners, directors, instructors, guest teachers, employees, assistants, and volunteers from any and all claims, liabilities, damages, or expenses arising from my child’s participation, except in cases of gross negligence or intentional misconduct.

In the event of an emergency, I authorize Doylestown Dance Centre staff to seek appropriate medical care for my child if I cannot be reached. I understand that I am responsible for any medical costs incurred.

I understand that registration fees are non-refundable unless the event is cancelled by Doylestown Dance Centre.

I have read and agree to the liability waiver and release above. I confirm that I am the parent/legal guardian of the student listed and authorize their participation in the Precision Jazz Master Class at Doylestown Dance Centre on July 1, 2026.
